Omni Health
DRAFT for legal review. Every [bracketed] item is a placeholder to confirm before publishing. These terms describe how the platform works today and are written to be fair and readable — but they are not legal advice and must be reviewed and approved before they take effect.

Terms of Use

The agreement between you and OmniHealth · Effective 1 June 2026

1. About these terms

OmniHealth (“OmniHealth”, “we”, “us”) is a health platform that lets individuals and families record, understand and share their health information, and lets health service providers and third-party developers offer tools and services within it. These Terms of Use are a legal agreement between you and [LEGAL ENTITY NAME, company number, registered address], the company that operates OmniHealth.

By creating an account or using OmniHealth, you agree to these terms and to our Privacy Policy, which explains how we handle your personal data. If you do not agree, please do not use the platform. If you use OmniHealth on behalf of an organisation, you confirm you are authorised to accept these terms for that organisation.

Some features have additional terms — for example, terms for health service providers, for developers using our API, or for a specific paid service. Where additional terms apply, they supplement these terms; if there is a conflict, the additional terms govern for that feature. [Confirm the set of supplementary terms — provider agreement, developer/API terms, research-contribution consent.]

2. Who can use OmniHealth

To hold your own account you must be able to enter a binding contract and be at least [minimum age — e.g. 16, or 18]. Families may keep records for children and for adults they care for, where they have lawful authority to do so; the account holder is responsible for how those records are used. [Confirm minimum self-managed age and the parental/guardian and carer authority model, consistent with the Privacy Policy.]

You are responsible for making sure your use of OmniHealth is lawful in your country. The platform is offered in several countries and some features, prices and currencies vary by country.

3. What OmniHealth is — and is not

A personal health journal you own and control. OmniHealth is a personal health journal — a place for you to gather, understand and share your own health information. It is not the official medical record kept by your GP, hospital or clinic, and it does not replace it. For a plain-language explanation, see Your health record, explained.

OmniHealth does not provide medical advice and is not a medical device. The platform, its content and its AI features are for information and organisation only. They do not diagnose, treat or prescribe, and they are not a substitute for advice from a qualified health professional. Always seek professional advice for medical concerns, and never disregard or delay it because of something you read or generated here.

In an emergency, do not use OmniHealth. Call your local emergency number or go to the nearest emergency department.

Where a health service provider delivers care to you through the platform, that care relationship is between you and the provider — not with OmniHealth. We provide the software; we do not practise medicine and are not responsible for the clinical decisions or advice of any provider (see §7).

4. Your account

  • Give accurate account and identity information and keep it up to date.
  • Keep your password and any recovery codes secret. You are responsible for activity under your account; tell us at security@omnihealth.solutions immediately if you suspect unauthorised access.
  • Because much of what you store is private health data, keep your recovery method safe — if account-recovery is not possible, some data may not be retrievable. [Confirm recovery model and any custodial/recovery guarantees before relying on this wording.]
  • Do not share, sell or transfer your account, and do not create an account for someone else without their authority.

5. Your content and your data

You own your content. The health-journal entries, notes, documents, images and other material you add (“your content”) remain yours. We do not claim ownership of it.

Licence to run the service. So that we can operate OmniHealth for you, you grant us a limited, non-exclusive licence to host, store, back up, process, display and transmit your content — only as needed to provide the platform, to follow your sharing instructions, and as described in the Privacy Policy. This licence ends when you delete the content or close your account, except for copies we must keep for legal reasons or that remain in routine backups for a limited period.

Your responsibility. You are responsible for the accuracy and legality of what you add, and for having the right to store and share it — especially information about other people. Do not upload content you are not entitled to.

Research is separate and optional. Contributing a pseudonymised copy of your journal for research is entirely optional, is based on your explicit consent, and can be withdrawn at any time. It is never a condition of using OmniHealth.

6. Sharing and permissions

You decide who can see your information and at what level — family, carers, health professionals, or third-party tools you enable. When you grant access you are authorising that recipient to view or act on the information you share, for as long as the access lasts. You can change or revoke access at any time, though people may have already seen or saved information shared with them.

When someone shares their record with you, use it only for the purpose it was shared and in line with the law and these terms. Do not copy, re-share or retain another person’s information beyond what they have authorised.

7. Health service providers

Health service providers on OmniHealth are independent of us. We do not employ them, do not provide healthcare ourselves, and do not endorse or guarantee any provider, service or outcome. Any clinical relationship, advice, treatment or transaction is between you and the provider, on their own terms. Verification badges indicate a check we have carried out but are not a guarantee. If you have a concern about care you received, raise it with the provider and the relevant professional regulator. [Confirm the provider agreement and the verification process referenced here.]

8. Developers and third-party services

OmniHealth offers an API (including FHIR interfaces) that lets approved developers build tools and services. Developer access is governed by separate developer/API terms and by the permissions you grant; a developer may only access data produced by a service you have enabled, and only with your consent. [Confirm developer/API terms location.]

Tools, services and links provided by third parties are the responsibility of those third parties, not us. Your use of them may be subject to their own terms and privacy notices.

9. AI features

OmniHealth offers optional AI features — including an AI feature that answers health questions about a patient. These features provide information and reasoning support only, under human oversight; they do not diagnose, make decisions with legal or similarly significant effects, or replace a qualified professional (see §3). Before any data is sent to our AI provider it is de-identified, as described in the Privacy Policy.

AI outputs can be incomplete or wrong. Do not rely on them for medical decisions, and always confirm anything important with a professional. Some AI features are charged per use (see §10).

10. Payments, wallet and pricing

  • Wallet & charges. Some features are paid and are charged to your wallet or chosen payment method. Prices and currencies vary by country and are shown before you buy.
  • Pay-per-use AI. Where a feature (such as an AI question) is charged per use, the charge is taken before the request runs and is automatically refunded if the request fails.
  • Card details. Card payments are handled by our payment provider; we do not store your full card details. [Confirm payment provider(s).]
  • Taxes, refunds & cancellations. Prices may be shown inclusive or exclusive of tax depending on your country. [Insert refund, cancellation and consumer-cancellation-rights policy, per each country of operation.]
  • Price changes. We may change prices and paid features; changes apply to future purchases, not to something you have already paid for.

11. Acceptable use

You agree not to:

  • break the law, infringe others’ rights, or use the platform to harm, harass, defame or impersonate anyone;
  • access, store or share another person’s data without authority, or upload unlawful, harmful or infringing content;
  • probe, scan, overload or try to bypass the security, access controls or rate limits of the platform, or use automated means to scrape or harvest data beyond the API and permissions we provide;
  • introduce malware, or attempt to gain unauthorised access to any account, system or data;
  • misuse AI features — for example to generate unlawful content or to seek to re-identify de-identified data;
  • resell or commercially exploit the platform except as expressly permitted.

We use automated protections against abuse and may block or throttle sources that trigger them.

12. Intellectual property

OmniHealth, its software, design, content we provide, and its names and logos are owned by us or our licensors and are protected by law. We grant you a limited, personal, non-transferable licence to use the platform in line with these terms. You may not copy, modify, reverse-engineer, or create derivative works of the platform except where the law allows or we give written permission. This does not affect your ownership of your own content (§5).

13. Availability, changes and early-stage features

We work to keep OmniHealth available and reliable, but we provide it on an “as available” basis. We may add, change, suspend or remove features, and we may carry out maintenance. Some features are new or in preview and may change or be withdrawn. We do not guarantee uninterrupted or error-free service. [If any service level or uptime commitment is offered to providers/developers, state it in their terms.]

14. Disclaimers and limitation of liability

To the fullest extent permitted by law, OmniHealth is provided “as is” and “as available”, without warranties of any kind, and we are not liable for reliance on information or AI outputs, for the acts of providers, developers or other users, or for indirect or consequential loss, or loss of data beyond our reasonable control. [Insert liability cap and the precise exclusions, reviewed per country.]

Nothing in these terms limits or excludes liability that cannot be limited by law — including liability for death or personal injury caused by negligence, for fraud, or for your statutory rights as a consumer. Your mandatory local consumer rights are not affected by these terms.

15. Suspension and termination

You can close your account at any time from your data & privacy rights page, where you can also download a copy of your data. We may suspend or end your access if you materially breach these terms, if required by law, or to protect the platform or its users; where reasonable we will give notice and a chance to put things right.

When your account ends, we handle your data as described in the Privacy Policy — deleting or anonymising it, except where we must retain certain records by law (in which case we restrict rather than delete). Terms that by their nature should survive — including §5 licence wind-down, §12, §14 and §16 — continue after termination.

16. Indemnity, changes and contact

Indemnity. To the extent permitted by law, you agree to cover reasonable losses we incur from your misuse of the platform or breach of these terms. [Confirm scope of indemnity per consumer-law limits.]

Governing law. These terms are governed by the law of [England and Wales — confirm governing law and courts, and any per-country consumer carve-outs for other markets]. For data-protection concerns you may also contact the UK Information Commissioner’s Office at ico.org.uk or your local supervisory authority.

Changes. We may update these terms; we will change the “last updated” date and, for material changes, tell you in advance. Continuing to use OmniHealth after changes take effect means you accept the updated terms.
