The agreement between you and OmniHealth · Effective 1 June 2026
OmniHealth (“OmniHealth”, “we”, “us”) is a health platform that lets individuals and families record, understand and share their health information, and lets health service providers and third-party developers offer tools and services within it. These Terms of Use are a legal agreement between you and [LEGAL ENTITY NAME, company number, registered address], the company that operates OmniHealth.
By creating an account or using OmniHealth, you agree to these terms and to our Privacy Policy, which explains how we handle your personal data. If you do not agree, please do not use the platform. If you use OmniHealth on behalf of an organisation, you confirm you are authorised to accept these terms for that organisation.
Some features have additional terms — for example, terms for health service providers, for developers using our API, or for a specific paid service. Where additional terms apply, they supplement these terms; if there is a conflict, the additional terms govern for that feature. To hold your own account you must be able to enter a binding contract and be at least [minimum age — e.g. 16, or 18]. Families may keep records for children and for adults they care for, where they have lawful authority to do so; the account holder is responsible for how those records are used. You are responsible for making sure your use of OmniHealth is lawful in your country. The platform is offered in several countries and some features, prices and currencies vary by country.
A personal health journal you own and control. OmniHealth is a personal health journal — a place for you to gather, understand and share your own health information. It is not the official medical record kept by your GP, hospital or clinic, and it does not replace it. For a plain-language explanation, see Your health record, explained.
OmniHealth does not provide medical advice and is not a medical device. The platform, its content and its AI features are for information and organisation only. They do not diagnose, treat or prescribe, and they are not a substitute for advice from a qualified health professional. Always seek professional advice for medical concerns, and never disregard or delay it because of something you read or generated here.
In an emergency, do not use OmniHealth. Call your local emergency number or go to the nearest emergency department.
Where a health service provider delivers care to you through the platform, that care relationship is between you and the provider — not with OmniHealth. We provide the software; we do not practise medicine and are not responsible for the clinical decisions or advice of any provider (see §7).
You own your content. The health-journal entries, notes, documents, images and other material you add (“your content”) remain yours. We do not claim ownership of it.
Licence to run the service. So that we can operate OmniHealth for you, you grant us a limited, non-exclusive licence to host, store, back up, process, display and transmit your content — only as needed to provide the platform, to follow your sharing instructions, and as described in the Privacy Policy. This licence ends when you delete the content or close your account, except for copies we must keep for legal reasons or that remain in routine backups for a limited period.
Your responsibility. You are responsible for the accuracy and legality of what you add, and for having the right to store and share it — especially information about other people. Do not upload content you are not entitled to.
Research is separate and optional. Contributing a pseudonymised copy of your journal for research is entirely optional, is based on your explicit consent, and can be withdrawn at any time. It is never a condition of using OmniHealth.
You decide who can see your information and at what level — family, carers, health professionals, or third-party tools you enable. When you grant access you are authorising that recipient to view or act on the information you share, for as long as the access lasts. You can change or revoke access at any time, though people may have already seen or saved information shared with them.
When someone shares their record with you, use it only for the purpose it was shared and in line with the law and these terms. Do not copy, re-share or retain another person’s information beyond what they have authorised.
Health service providers on OmniHealth are independent of us. We do not employ them, do not provide healthcare ourselves, and do not endorse or guarantee any provider, service or outcome. Any clinical relationship, advice, treatment or transaction is between you and the provider, on their own terms. Verification badges indicate a check we have carried out but are not a guarantee. OmniHealth offers an API (including FHIR interfaces) that lets approved developers build tools and services. Developer access is governed by separate developer/API terms and by the permissions you grant; a developer may only access data produced by a service you have enabled, and only with your consent. Tools, services and links provided by third parties are the responsibility of those third parties, not us. Your use of them may be subject to their own terms and privacy notices.
OmniHealth offers optional AI features — including an AI feature that answers health questions about a patient. These features provide information and reasoning support only, under human oversight; they do not diagnose, make decisions with legal or similarly significant effects, or replace a qualified professional (see §3). Before any data is sent to our AI provider it is de-identified, as described in the Privacy Policy.
AI outputs can be incomplete or wrong. Do not rely on them for medical decisions, and always confirm anything important with a professional. Some AI features are charged per use (see §10).

We use automated protections against abuse and may block or throttle sources that trigger them.
OmniHealth, its software, design, content we provide, and its names and logos are owned by us or our licensors and are protected by law. We grant you a limited, personal, non-transferable licence to use the platform in line with these terms. You may not copy, modify, reverse-engineer, or create derivative works of the platform except where the law allows or we give written permission. This does not affect your ownership of your own content (§5).
We work to keep OmniHealth available and reliable, but we provide it on an “as available” basis. We may add, change, suspend or remove features, and we may carry out maintenance. Some features are new or in preview and may change or be withdrawn. We do not guarantee uninterrupted or error-free service. To the fullest extent permitted by law, OmniHealth is provided “as is” and “as available”, without warranties of any kind, and we are not liable for reliance on information or AI outputs, for the acts of providers, developers or other users, or for indirect or consequential loss, or loss of data beyond our reasonable control. Nothing in these terms limits or excludes liability that cannot be limited by law — including liability for death or personal injury caused by negligence, for fraud, or for your statutory rights as a consumer. Your mandatory local consumer rights are not affected by these terms.
You can close your account at any time from your data & privacy rights page, where you can also download a copy of your data. We may suspend or end your access if you materially breach these terms, if required by law, or to protect the platform or its users; where reasonable we will give notice and a chance to put things right.
When your account ends, we handle your data as described in the Privacy Policy — deleting or anonymising it, except where we must retain certain records by law (in which case we restrict rather than delete). Terms that by their nature should survive — including §5 licence wind-down, §12, §14 and §16 — continue after termination.
Indemnity. To the extent permitted by law, you agree to cover reasonable losses we incur from your misuse of the platform or breach of these terms. [Confirm scope of indemnity per consumer-law limits.]

Changes. We may update these terms; we will change the “last updated” date and, for material changes, tell you in advance. Continuing to use OmniHealth after changes take effect means you accept the updated terms.
